Summary Vasovagal syncope during central neuraxial blockade is a recognised phenomenon. We report a case of syncope, which was preceded by an increase in the amplitude of the plethysmograph trace. We review the physiological changes leading to such an episode and the role of plethysmographic pulse oximeter waveform variation monitoring in the prediction of syncope.
