Summary We describe the case of a patient presenting with paraplegia secondary to a thoracic epidural abscess. In the post‐operative period his management was complicated by life threatening pulmonary embolism. The case highlights the complexity in medical decision‐making relating to managing massive pulmonary embolus in the immediate post‐operative period following invasive spinal surgery.
