Summary A 4‐month‐old, 6.7kg, male with 22q11.2 microduplication, coarctation of the aorta, aortic arch hypoplasia, and bicuspid aortic valve underwent aortic arch advancement as a neonate. He presented for follow‐up transthoracic echocardiography under sedation. Following administration of 2.2 μg.kg ‐1 nasal dexmedetomidine he became very fussy and both feet became hyperaemic and swollen in a “stocking” distribution. Bilateral residual bruising occurred. There were no long‐term sequelae.
