Tissues of apical granulomas at the exacerbation and remission stages before and after the treatment with the use of transcanal electrogranulotomy (TCEGT) have been studied by the methods of histological and clinical X-ray investigations and nonparametric statistics. The content of polymorphonuclear leukocytes at the exacerbation doubled compared to the remission (p < 0,05). After the treatment at the exacerbation stage (87 teeth) and beyond it (116 teeth), the activation of macrophages and fibroblasts was observed, as well as the appearance of Malassez's epithelial rests. The reconstruction of the bone structure has been found by the X-ray investigation in 97,6% of patients in the period up to 5 years. The treatment of granulomatous periodontitis with TCEGT is possible at any stage.
