We report an elderly patient suffering from type 2 diabetes mellitus for over 10 years, under conventional anti-diabetic therapy for a number of months and not adequately responsive. The patient, in unstable glycemic compensation, is obese, hypertensive and presents microalbuminuria. He is also affected by several co-morbidities (Diabetology)
