The author observed a case of severe vomiting with indefinite abdominal pain in a 6-year-old child, the son of a doctor. The picture of the disease was very similar to an acute attack of appendicitis, but the strong smell of acetone in the patient's exhaled air was disconcerting. Urinalysis also revealed the presence of acetone in the urine.
