Although rectus sheath hematoma (RSH) is rarely observed, it can be an important cause of abdominal pain. The disease is more common in elderly people, particularly if they are taking anticoagulants. Misdiagnosis is frequent and more likely if the presentation is atypical. It is important to consider RSH in elderly patients with an abdominal mass or spontaneous ecchymosis. In this article, we describe a case of RSH in an elderly patient.
