External septorhinoplasty was performed on 15 patients over n 12‐year period in a general pediatric otolaryngology practice. It is the operation of first choice when there is significant dorsal nasoseptal pathology. Good cosmetic and functional results are obtained by full dissection of the nasoseptal pathology, judicious resection of thickened deformed cartilaginous and osseous tissues, and repositioning of a suitable nasoseptal fragment anteriorly in the midline.
