The intranasal ethmoidectomy operation can cause disastrous complications: blindness and death. It is usually performed by using the middle turbinate as a guide. Sometimes prior disease or surgery may cause this to vary from its usual position. Perhaps a better guide is the bone of the medial wall of the maxillary sinus since this always lies in the same vertical plane or just medial to the lamina papyracea. Staying medial to this line should protect the orbit, when doing an intranasal ethmoidectomy.
