Paecilomyces is a fungus only recently recognized as a human pathogen. We describe the only known case of sinusitis to date due to such a fungus. The histologic characteristics of paecilomyces are presented, and based on a review of fungal sinusitis and a disease‐free 30 month follow‐up, the treatment of this entity is suggested to be conservative local surgical debridement and aeration.
