The possible underlying causes of temporal lobe herniation into the radical mastoid cavity are discussed, and the surgical principles and operative techniques for its correction are detailed and illustrated. The method herein described has been found to be effective and reliable in permanently correcting the dura and tegmen bone defect. A representative case history is presented.
