The goal of tympanoplasty is hearing restoration. One prerequisite for obtaining this objective is a mucosal‐lined air‐filled middle ear space. Silastic and Supramid sheeting are used in selected cases to accomplish this result. In the absence of middle ear mucosa the operation must be performed in two stages. The otologist must have a thorough knowledge of the principles of middle ear surgery and the causes of failure. Careful execution of precise surgical techniques should result in satisfactory hearing in the majority of cases.
