We report a case of an adult male with continued hypoxia and polycythemia after repair of significant pulmonic valve stenosis. A large ostium secundum atrial septal defect was found to be causing these symptoms. We discuss the workup at his initial visit and return to hospital, the diagnosis, and treatment of the pulmonic stenosis and atrial septal defect. We discuss the sudden onset of symptoms in this adult patient and the importance of further screening for a right-to-left shunt through an atrial septal defect in an adult patient with pulmonic stenosis.
