A 10‐year‐old neutered male shih tzu was presented due to a history of rectal tenesmus, dyschezia and constipation for the previous two months. Physical assessment disclosed swelling on the left ischiorectal fossa, lateral to the anus. The swollen area was fluctuant, and the content was reducible. Rectal examination revealed rectal dilation and deviation. Perineal area palpation and rectal examination in addition to the history and clinical signs conduced to a diagnosis of perineal hernia. The patient was treated surgically, a herniorrhaphy being performed. A cone‐shaped polypropylene mesh implant was used as herniorrhaphy technique. Clinical signs were successfully controlled with the surgical procedure. No complications or hernia recurrence have been reported to date.
