Three causes of reduced output of orthovoltage radiotherapy units have recently been reported in this journal, each being due to faults external to the tube insert. A further incident is now reported where the output was reduced by 30% due to the tungsten target foil becoming partially detached from the anode, while the unit continued to operate normally. The possible cause and the prevention of a repeat occurrence are discussed.
