Key Content Following the pause on the use of mesh, women are opting more for non‐mesh‐based options for stress urinary continence (SUI) surgery. Even after any re‐introduction of mesh procedures, women may still prefer non‐mesh options for SUI surgery.Learning Objectives To know the current situation regarding stress urinary incontinence surgery across the UK, the recent guidelines provided by the National Institute for Health and Care Excellence (NICE) and the recommended non‐mesh surgical alternatives for SUI. To know what training is required for non‐mesh continence procedures. To understand the issues in clinical governance and ethics associated with non‐mesh surgery for SUI.Ethical issues Should we routinely offer surgeries that are potentially more invasive than mesh tapes without offering a full range of alternatives that include mesh tapes? The need for informed consent and individualised counselling, including evidence that is available and the lack thereof when applicable. The duty of candour when surgeons start their training in surgeries that have a steep learning curve.
