Key content This article summarises the current management and outcome of gynaecological cancers (cervical, endometrial, vulvovaginal, ovarian and fallopian tube cancer) in relation to pregnancy. Pre‐existing gynaecological cancer and pregnancy planning is discussed, as well as prepartum diagnosis with or without intrapartum recurrence, intrapartum diagnosis and postpartum management.Learning objectives To understand prepregnancy planning with involvement of the multidisciplinary team and counselling. To understand how to approach gynaecological cancers in relation to pregnancy inclusive of patient factor and stage of malignancy. To be able to give optimal management of these patients to provide the best outcome for the mother and baby.Ethical issues Sanctity of life – balancing optimal maternal therapy versus fetal health. Termination of pregnancy and personal beliefs. Utility – benefit over harm and the quality of life for both mother and baby given certain outcomes.
