A case report of an 8 year old patient with a large odontogenic keratocyst that was preventing the eruption of several permanent mandibular teeth. A presentation like this led to the suspicion of a mild form of Gorlin Goltz syndrome. Through careful planning with the consultant orthodontist and maxillofacial surgeon, the patient now presents age 16 with a full set of healthy adult teeth, which are both aesthetically and functionally pleasing.
