Ranula seldom happens as a complication secondary to the surgery surrounding the sublingual gland. Here, we reported that a rare case of sublingual gland ranula after tongue reconstruction with radical forearm flap, saliva extravasation after trauma to the sublingual gland or the ducts was responsible for the occurrence of this ranula.
