Recurrent bacterial meningitis is not a frequent clinical condition. We describe here the case of a woman with recurrent pneumococcal meningitis caused by the leakage of CSF due to an occult middle fossa encephalocele related to chronic otitis media. To detect an extremely small encephalocele, detailed evaluation by bone CT and RI cisternography are very useful. In the treatment of recurrent bacterial meningitis, clinicians should examine for the presence of a small encephalocele early and perform an appropriate surgical therapy, because the condition could be fatal.
