We report a patient with granulomatosis with polyangiitis (GPA) developing a major ischemic stroke treated with mechanical thrombectomy. A 65‐year‐old man developed an occlusion of the left internal carotid artery (ICA) during admission for evaluation of GPA. He was treated with intravenous thrombolytic therapy followed by mechanical thrombectomy. This is the first report of a GPA treated with mechanical thrombectomy.
