A man with primary brainstem infarction developed the “locked‐in” syndrome. Magnetic resonance imaging (MRI) demonstrated occlusion of the vertebral and basilar arteries associated with structural changes in the pons and midbrain. Cerebrospinal fluid analysis was significant for elevated protein content and high titers of myelin basic protein.
