Although symptomatic vasospasm complicating aneurysmal subarachnoid hemorrhage is well known, vasospasm rarely follows nonaneurysmal subarachnoid hemorrhage. This reports a patient who suffered delayed neurological deterioration following a parietal lobe hemorrhage that extended into the subarachnoid space. Transcranial Doppler findings were consistent with middle cerebral artery vasospasm, which was confirmed by angiography.
