Computed tomography and magnetic resonance imaging of 2 patients with back pain demonstrated a nonossified mass in the epidural space anterior to the thecal sac and in the midportion of the thoracic or lumbar region of the spine, most prominent at the middle of the vertebral bodies. Imaging characteristics were compatible with hypertrophy of the posterior longitudinal ligament. This disease appears to be different from ossification of the posterior longitudinal ligament.
