A retrospective review of 285 very‐low‐birth‐weight infants with periventricular‐intraventricular hemorrhage revealed that 20 infants developed posthemorrhagic dilation of the fourth ventricle. Evaluation of the survivors for 16 months (mean follow‐up period) showed a statistically significant increase in severe disability and in impaired cognitive development, compared with an equal number of control subjects matched for birth weight and grade of hemorrhage. These findings indicate that very‐low‐birth‐weight infants who develop dilation of the fourth ventricle following periventricular‐intraventricular hemorrhage have a greater likelihood of a poor outcome than those who do not have this complication. The predictive function of this sign appears to be independent of the grade of hemorrhage.
