The most widely used technique for chronic dissection of the aortic arch and thoracoabdominal aorta is the “elephant trunk” (ET). A technique, in which an original modification of the ET technique was necessary to avoid visceral malperfusion due to the obliteration of the patent false lumen from where splanchnic arteries arose, is presented. doi: 10.1111/jocs.12267 (J Card Surg 2014;29:364–367)
