Treatment of Atrial Fibrillation in three Racehorses

SUMMARY Three young Standardbred pacers with atrial fibrillation were treated with quinidine sulphate (QS) by stomach tube. They were given 10 g QS every 2 hours until cardioversion was achieved. Total doses varied between 20 and 30 g. No premedication was given nor any follow up treatment after return to sinus rhythm. All horses were given 3 months rest after the treatment, and when electrocardiographed then, and at 6 months, showed normal sinus rhythm. As all 3 horses won races after cardioversion and showed subsequent normal electrocardiograms it seems likely that atrial fibrillation can occur without detectable pathological changes in the heart. RÉSUMÉ Trois jeunes ambleurs atteints de fibrillation auriculaire furent traités à l'aide de sulfate de quinidine administré par cathétérisme naso oesophagien. On leur administra dix grammes de sulfate de quinidine toutes les deux heures jusqu'au rétablissement d'un rythme sinusal normal. Les doses totales administrées furent de 20 à 30 grammes. Aucun traitement complémentaire ne fut donné ni avant ni après le retour au rythme normal. Les trois chevaux bénéficièrent d'un repos de trois mois après le traitement et leur ECG après trois mois et six mois furent normaux. Tous les trois coururent ensuite et leurs tracés ECG ultérieurs ne montrèrent pas d'altération. On pense ici que chez le cheval la fibrillation auriculaire peut se produire sans qu'on puisse déceler un changement pathologique du myocarde. ZUSAMMENFASSUNG Drei junge amerikanische Passgänger mit Vorhofflimmern wurden mit Chinidinsulfat per Nasenschlundsonde behandelt. Sie erhielten alle zwei Stunden 10 g Chinidinsulfat bis zum Eintritt eines Sinusrhythmus. Die gesamten Dosen variierten zwischen 20 und 30 g. Die Pferde erhielten keine Praemedikation und auch keine weitere Nachbehandlung. Alle Pferde wurden jedoch für drei Monate nach der Bahandlung ruhiggestellt und zu diesem Zeitpunkt, sowie nach 6 Monaten, elektrocardiographisch untersucht. Weil alle drei Pferde nach der Behandlung Rennen gewannen und später normale elektrocardiographische Befunde aufwiesen, ist anzunehmen, dass Vorhofflimmern ohne nachweisbare pathologische Herzveränderungen auftreten kann.
