SUMMARY A Centralized Transfusion Service (CTS) utilizes an integrated laboratory‐based approach to the delivery of transfusion medicine services within a region. The CTS model has emerged as probably the most cost‐efficient method for the delivery of these services. Its positive impact on both operational and component costs exceeds the level of hospital savings typically achieved by merely negotiating a “cheaper” red cell with the blood provider. Even the regional blood provider benefits from the superior management of the community's blood supply. Hospitals and hospital networks seeking to reduce transfusion‐associated costs should consider a CTS model.
