SUMMARY Cardiovascular disease accounts for a majority of the deaths each year in westernized countries. Anemia, with its obligatory increase in cardiac output, may present a special risk in these patients. A review of the physiology of acute and chronic anemia as well as the physiology of myocardial ischemia is presented. A review of the literature regarding the risks and recommendations for therapy in this group of high‐risk patients is presented.
