The use of traditional glaucoma drainage devices (GDD) plays a vital role in the management of patients with refractory glaucoma. In a recent US survey the number of GDD implants had increased from 7,788 in 2003 to 12,021 in 2012. However, GDD implantation can be difficult‐ due to either the disease pathology or scarring from previous surgery. This presentation will discuss options for when the GDD has not been able to sufficiently reduce the intra‐ocular pressure or completely failed. A management step ladder will be looked at to review, medical, surgical and laser options in high risk recalcitrant glaucoma patients.
