Summary In this contribution we would like to give an overview in the spectrum of pseudophakic and phakic toric lenses currently on the market. The audience will be guided how to select the proper lens for the indivual patient. Perioperative marking, surgical technique, intraoperative positioning and validation of lens alignment will be addressed in detail. Techniques are shown how to prevent postoperative rotation, decentration or tilt of the lens. In a last step, we will demonstrate how and when to re‐position or re‐align (in axis) the pseudophakic or phakic toric lens implant in case of decentration, tilt or axis rotation. The add‐on value of modern intraoperative assistance / guidance functions for proper positioning of the toric lens implant or re‐positioning in terms of complication management is shown.
