Patients with unvoluntary eye movements frequently consult an ophthalmologist. A correct diagnosis of the type and etiology is important. A clinical approach with selected technical examinations is mandatory for a correct diagnostic approach. Isolated ophthalmological and combined neurological elements can be identified to guide the further approach. Conclusions A practical work‐up of a patient presenting with nystagmus will be presented. Frequently the collaboration with the neurologist is important.
