The endothelial cell density was studied in 162 penetrating corneal grafts with a post‐operative time from 1 to 125 months. The central cell density was found to decrease markedly in the post‐operative period from more than 2000 cells/mm 2 in the early months to less than 1000 cells/mm 2 4 years after the operation. After this, no further decrease was detected. Cell densities obtained from a peripheral area on the graft were also observed to decrease, indicating that the total number of endothelial cells decreased in the post‐operative period. This study indicates that the post‐surgical influence on the graft endothelium is probably of greater importance than previously recognized, and must be accounted for when, on the other hand, the surgical influence is evaluated.
