This article describes the treatment of a patient with a history of a hypersensitivity reaction to Xylocaine with bupivacaine. The adverse reaction to Xylocaine was further documented by a positive skin test. The skin test using a progressive challenge protocol is described as a means to prevent adverse local anesthetic reaction and to select an appropriate anesthetic.
