What is the goal of treatment of diabetes? Many would primarily recommend glucose lowering; others would add lipid and blood pressure control. One may give specific levels of parameters: glucose or A1c; lowdensity lipoprotein–cholesterol or apolipoprotein B; systolic or diastolic blood pressure. These are, however, not treatment goals in any fundamental sense; rather, glycemia, lipidemia, and hypertension, as well as intermediate measures, such as albuminuria, glomerular filtration rate, carotid arterial thickness, coronary calcification, retinal microaneurisms, and nerve conduction (and many similar measures could be proposed) are simply steps on the road to the true goal of the treatment of diabetes: prevention of the complications of diabetes. With the inauguration of the Journal of Diabetes, we hope to create a new vehicle for exploring both clinical and basic understanding of the mechanisms underlying the development of diabetes and its complications, as well as for developing successful therapeutic approaches. This is an immense undertaking and we are fully aware of the many other journals helping advance knowledge in the field. Our journal is designed to have a unique mix of Eastern and Western participation, undertaking to bring together the ‘‘established’’ centers of diabetes research in the West with new centers not only in China, but also throughout Asia and the developing world. Diabetes is a disease that respects no borders, affecting all of humanity, and, in joining the effort to effectively communicate ideas and research to a practicing worldwide audience via both the print medium and new efforts in Internetbased publication, we look forward to assisting in the endeavor to reduce the burden of this complex condition.
