Ankylosis of the temporomandibular joint is a very debilitating condition. A case report of a 12‐year‐old girl with ankylosis between the mandibular notch and the skull base is presented. Trauma was identified as the probable cause. Al Kayat and Bramley approach followed by osteotomy and lateral swing of the zygomatic arch gained extraoral access to the site. Osteotomy of the bony mass was done between the mandibular notch and the skull base. The patient had good mouth opening at 1‐year follow up.
