This report describes the case of a 12‐year‐old girl who presented with diffuse enlargements of the anterior gingiva in both jaws. The clinical diagnosis was an inflammatory hyperplasia, but gingivectomy samples from all involved sites showed similar histological features consistent with odontogenic fibroma. Another peculiar finding was generalised mottled enamel. This is the fifth case of a diffuse clinical variant of peripheral odontogenic fibroma documented in the literature.
