A framework is given for thinking about the clinical approaches to sexual incompatibilities. It is simplified by breaking down into the three major stages of marriage. Early marriage presents the facets of drive level differences, of sexual ignorance, fatigue states, and paired‐relation images. The sexual complaint is rarely presented as such. Middle marriage is laden with a host of troubles which will present some sexual face. Personal incompatibilities are well developed, progressive alienation is occuring, preoccupation with career or child raising is prominent, and frank psychiatric states come into full bloom. Sexual complaints are readily voiced at this stage. Advanced marriage is marked by the aggravated incompatibilities, the increasing occurence of disease states and both the conditioned libido loss and that induced by medications. The specific problems, and their possible solutions are more readily identified and dealt with within this kind of framework.
