An elderly female West Highland white terrier was presented with a history of recent enlargement of an inguinal swelling with concomitant lethargy and inappetence. The radiographic and ultrasonographic findings were suggestive of a pyometra incarcerated within an inguinal hernia. Following routine ovariohysterectomy and hernia repair, the bitch made an uneventful recovery.
