. Alzheimer's disease has a long early phase in which limited pathological changes accumulate without apparent functional consequences. Once pathology exceeds a certain threshold, clinical features arise, but this threshold seems to differ from patient to patient, reflecting individual differences in ‘;brain reserve’. This review discusses the concept and possible determinants of brain reserve, and their implications for the diagnosis of Alzheimer's disease.
