Summary A review on pathogenetic factors related to idiopathic hypercalcemia is presented. Certain discrepancies between the clinical picture of hypervitaminosis D and that of idiopathic hypercalcemia are pointed out. On the basis of metabolic and clinical data, obtained in cases of idiopathic hypercalcemia on one side and present knowledge of fundamentals of calcium metabolism on the other, the hypothesis is advanced that idiopathic hypercalcemia is caused by a disturbance of the homeostatic mechanism controlling the intestinal absorption of calcium rather than by a state of vitamin D overactivity.
