Summary 1 . A brief review is given of the diagnostic approach in neurology as practised in this department. 2. A case is described where pronounced spinal automatic reflexes were to be recorded. The disorder in this case was a hydrocephalus internus. When this condition was neurosurgically adjusted the reflexes returned to normal. The word “spinal” should accordingly only be used cum granu salis when these reflexes are concerned. 3. Attention is called to the possibility that an originally partial obliteration of the aqueduct may be turned into a complete obliteration by means of the pressure exerted upon midline‐structures by the distended cerebral hemispheres. 4. The strikingly short distance between inion and the processus spinosus of the epistropheus may be suggestive of the aqueduct as the seat of the obliteration.
