Summary An exchange transfusion was performed in an infant with neonatal thrombocytopenic purpura. Both the infant and the mother presented some unusual features, which are briefly discussed. Although the disease is self‐limited, the high mortality during the first days after birth indicates the use of effective therapy as soon as possible. Compared to other methods of treatment, exchange transfusion seem to be the best in several respects. It is suggested that women with idiopathic thrombocytopenic purpura should be delivered in hospitals with exchange transfusion units and laboratories for haemato‐logical serology.
