This case report details the treatment of a maxillary central incisor with an incomplete apex following trauma to the tooth. An apexification procedure was carried out and the patient was asymptomatic until the time of a subsequent accident. The tooth was fractured in the cervical third of the root, extracted and examined microscopically following decalcification. An interesting pattern of apical closure was noted, where in addition to an apical cementum bridge, a layer of cementum had been deposited along the walls of the root canal to the junction of the middle and cervical thirds.
