Long‐term calcium hydroxide treatment of a tooth with iatrogenic root perforation and lateral periodontitis

This case report describes the treatment of a lower right canine with a periapical radiolucency and a radiolucency associated with a lateral perforation that was made 10 years previously, during an apicoectomy procedure on the adjacent lateral incisor. Long‐term calcium hydroxide treatment was carried out. After 3 months the apical radiolucency was greatly reduced and after 1 year was completely resolved. After 3 years a hard tissue barrier had formed across the lateral perforation defect, but at no time during the treatment period did the inflammatory lesion adjacent to the perforation improve. It is suggested that epithelial downgrowth and possibly periodontal pocket formation had occurred, allowing irritants from the gingival sulcus to sustain the inflammatory reaction in the perforation area.
