The authors present the case of a patient with symptomatic sinus bradycardia. Implantation of a rate responsive atrial pacemaker improved the exercise capacity of the patient, but exercise intolerance was still intermittently present. Only Holter monitoring could reveal the development of AAI, R pacemaker syndrome that led to the symptoms. Reprogramming of the pacemaker eliminated the phenomenon.
