A 42‐year‐old dialysis patient presented with congestive heart failure. A large, calcified mass on the posterior mitral leaflet with an opposing anterior leaflet perforation, and a diastolic jet extending into the left ventricular outflow tract was noted on Doppler echocardiography. The patient underwent mechanical mitral valve replacement and surgical specimen confirmed these findings with no evidence of endocarditis.
