A 25‐year‐old man who had undergone aortic valve replacement 4 months earlier presented with one episode of syncope. A transthoracic echocardiogram revealed a saccular structure was below the aortic valve annulus and a small vegetation was on the aortic valve prosthesis; blood cultures were positive for Staphylococcus epidermidis. Thoracic computed tomography scan, two‐dimensional transesophageal echocardiogram (TEE), and three‐dimensional TEE demonstrated a combination of two pseudoaneurysms due to severe endocarditis, pseudoaneurysm of the ascending aorta, and pseudoaneurysm of the mitral‐aortic intervalvular fibrosa.
