A 57‐year‐old white male status post St. Jude aortic prosthesis, who 10 years later presented with congestive heart failure was found to have a fistula between the left ventricular outflow tract and left atrium at the mitral‐aortic intervalvular fibrosa with no evidence of endocarditis or trauma.
