We describe a patient with severe aortic regurgitation in whom transesophageal echocardiography coupled with cardiac catheterization and pathological examination confirmed the diagnosis of a quadricuspid aortic valve, anomalous coronary artery, and aneurysm of the noncoronary sinus of Valsalva.
